Before submitting this form, you will need to prepare an Ancestry Chart (also known as a Pedigree Chart) with a Family Group Sheet for each couple of interest.  You should also have scanned copies of supporting documents such as certificates.

There are 3 ways of preparing an Ancestry Chart or a Family Group Sheet:

  1. Generate it from your genealogy software and save it as a PDF.
  2. Download a blank form, print it, fill it in by hand, scan it, and save it as PDF or JPG.
  3. Download a blank form, enter the details into the PDF, and save the PDF.

Non-Members Consultation Service

  • An appointment can be made for one hour with a research consultant.
  • Before booking an appointment, you may wish to visit the GSV and ask for the assistance of the Research Assistants.
  • Please advise time-frame and area of your research when requesting an appointment.
  • Please bring copies of all relevant documents you hold.
  • The cost is $50.00 for a one hour consultation.

Please use meaningful names for your files to help the Research Team keep track of everyone's files.

Please attach your Ancestry Chart and a Family Group Sheet for each couple of interest together with any associated documents.

You need to upload one Ancestry Chart and at least one Family Group Sheet.  You should also upload any supporting documents such as certificates.  You can upload up to 20 files.
